Bay Harbour Med Spa has received dozens of requests to write more articles about PCOS. We write about PCOS, because it is a condition that is very personal to us. Polycystic Ovarian Syndrome (PCOS) is a condition experienced by some female teens and young women. It usually presents itself with menstrual abnormalities together with other features arising from excessive androgens (hyperandrogenism), although androgens are the hormones associated with masculine features, all women have low levels of male hormones, and all men have low levels of female hormones.

PCOS is associated with excess or higher than normal levels of male hormones in women.

Features of PCOS may develop at any age:

  • If arising during childhood it usually causes premature puberty
  • If arising during teenage years it is usually exhibited by excess hair and/or by menstrual abnormalities
  • If arising during early adulthood or middle life it usually presents by infertility and/or by glucose intolerance and
  • If arising during later life it usually presents with diabetes and cardiovascular disease. If you have symptoms, it is essential that you seek medical advice and investigation to confirm or eliminate PCOS.

Excess and Unwanted Hair

Women with PCOS typically experience symptoms associated with excess androgen levels including excess hair growth (hirsutism) and acne. An increased growth of coarse hair can occur on the face, nipple, chest, thighs or in the middle of the lower abdomen. For most women the appearance of excess and unwanted hair is distressing, and affects their self esteem and body image.

Other Symptoms

Some women with PCOS also exhibit acne.  We usually associate acne with adolescence, women who continue to get acne well after this time find it frustrating and embarrassing. In addition, the moderate to severe acne usually associated with PCOS can result in unsightly scarring.

A further common symptom of PCOS is obesity, with up to 70% of women being affected to some extent. Women with PCOS tend to gain weight in the abdominal region rather than on the buttocks or thighs, the usual areas woman “put on” weight.

Often the weight gain is related to ‘insulin resistance’, a condition that is typically associated with PCOS, Medical practitioners alert us that some women with PCOS who have insulin resistance are of normal weight, so regular testing is recommended and the absence of weight gain should not be considered as cause for complacence in managing PCOS.

Insulin is a hormone that allows the conversion of glucose (sugar) into energy. When insulin resistance occurs, the body produces more of the hormone to compensate; the elevated insulin level stimulates the body’s fat cells to make fat from nutrients and to store it, causing weight gain. A high level of insulin may also cause further increase in the production of androgens by the ovaries.

Women with PCOS may also experience menstrual disturbances: very light or heavy bleeding; infrequent periods; or absence of periods. Most women with PCOS started their periods at a normal age but they are irregular and over time may disappear altogether. Irregular periods are normal during adolescence these menstrual disturbances are usually not cause for concern and consequently they are not investigated further.

Women with irregular periods and/or acne may be prescribed an oral contraceptive pill which eliminates a ‘true’ period altogether. With the menstrual-related symptoms no longer apparent, consequently PCOS may go undetected. We are not big advocates of birth control pills for the treatment of PCOS, but you should discuss this option with your gynecologist. For some women, PCOS is only diagnosed following investigations for problems with conception.  It is estimated that PCOS is present in more than 70% of women with infertility arising from failure to ovulate.

Women with PCOS are also at risk of endometrial hyperplasia, a condition in which the lining of the uterus is over stimulated by estrogen and becomes overgrown. Endometrial hyperplasia is strongly associated with irregular ovulation.

Medical Treatment and Diagnosis

Many of the symptoms of PCO include: obesity, acne and hirsutism. At Bay Harbour Med Spa we occasionally identify women who have not yet consulted their medical practitioner about symptoms consistent with PCOS, whilst treating excess and unwanted hair we are also able to encourage them to consult their medical practitioner so as to investigate the underlying causes and confirm or exclude PCOS as a cause. If the symptoms improve with treatment of symptoms of PCOS (i.e. acne helped by Crystal Microdermabrasion such as offered at Bay Harbour Med Spa) the underlying condition of PCOS may not be detected until further problems present. It is important to discuss symptoms with your medical practitioner and investigate the possibility of PCOS rather than delaying and consequently exposing yourself to the risk of long term health problems like diabetes and cardiovascular disease.

Diagnosis of PCOS usually involves an ultrasound of the ovaries and blood tests to determine hormone levels, your medical practitioner will also take a thorough medical history (if not already recorded).

Treatment for PCOS depends on the symptoms presenting and whether you are trying to get pregnant. 

You may also benefit from advice and support from the various PCOS support groups. Support groups provide current and specific information and allow women to share their experiences. Being able to talk to someone who has similar problems can help reduce women’s feelings of isolation. Acne is caused by the action of the androgens on the hair follicles and sebaceous glands in a persons susceptible to acne.  Acne begins when a hair follicle produces cells that stick together so tightly together that they cannot be shed. These cells mix with bacteria and form a plug causing the pore to get blocked. This type of lesion is called a comedone.

The sebaceous gland continues to produce sebum (oil). Frequently the follicle bursts or leaks, forming a papule or pimple. As the follicle releases its contents into the surrounding tissue, inflammation occurs. This is why the surface surrounding the pimple is reddened and swollen. White blood cells enter the tissue, causing it to liquefy, forming pus.

Acne vulgaris consists of four types of blemishes:

  1. Pustules ( pimples with pus at the top)
  2. Comedones ( the closed kind are white heads, the open kind are black heads)
  3. Deep cysts
  4. Abscesses

Acne is a common skin disease that is brought about by the plugging and inflammation of the hair follicle. It manifests itself as papules, pustules or comedones. Usually, acne is found on the face, chest, and back. In certain severe cases acne may produce scarring.

Acne vulgaris is the most common skin condition affecting those in the age group that extends from the early teens to the mid-thirties.  One of the biggest myths about medical treatments geared to treat acne is that these medical treatments cure the symptoms; they do not.  Medical treatments are useful in controling the symptoms.

What are some available treatments  used to treat acne?

Bay Harbour Med Spa summarizes the treatments as the following; some patients are treated with antibiotics which retard the bacterial growth and prevent comedo, papule, and pustule formation. If the acne is severe, cysts will develop. Cysts can be injected with cortisone, typically this is done by a dermatologist.  Treatment of acne depends upon the type of acne the patient has. Mild cases are treated by any of the following: acne facials ( should be done by a trained aesthetician, we recommend that he/she have at least ten years experience), microdermabrasion treatments ( Bay Harbour Med Spa was South Florida’s pioneer of microdermabrasion treatments and customized it’s own microdermabrasion treatment- widely knows as Crystal Microdermabrasion TM), topical treatments such as benzoyl peroxide ( although, it is our experience that BP is too drying and unless the product is customized and blended specific to your problem you may want to use BP cautiously or not at all), antibiotics, or tretinoin ( Retin-A); the latter prescribed by a dermatologist.

Patients with severe acne may be treated with isotretinion ( Accutane) which is a derivative of Vitamin A. Accutane has some severe side-effects, we suggest that you do the research and have a thorough medical history performed at your dermatlogist’s office before commencing treatment. Crystal Microdermabrasion TM is Bay Harbour Med Spa’s leading treatment against the fight of acne as well as acne scarring. We do on occassion recommend our Crystal Microdermabrasion TM treatments in conjunction with collagen injections for severe acne scarring.

Can makeup cover acne marks?

Unfortunately, the pitted acne marks that result from acne are difficult to disguise with makeup. Cover creams can be useful if they are applied sparingly to the areas taht are discolored by scarring. 

What type of makeup should not be used on acne sufferers?

Oil based, opaque makeup should never be used on patients with active acne because it is greasy and can interfere with topical medication. One other very important factor…………………the sponges used to apply makeup harbor bacteria causing acne; if you do  apply makeup, use disposable sponges. Waterbased makeup is more appropriate. You should be aware that waterbased makeup will minimally cover acne or acne scarring, but it is healthier for this type of condition.
